Title: Schematic for OTAX - VLX91 Switch

Hello
I have  a Cold Sweat Neck and a Miracle Man Bridge humbucker. They are both 4 conductor hookups. I want to use a VLX91 OTAX switch (5way)
Anyone has got a schema for that?
