I've just made some modifications to my Pacifica 112. I changed the volume pot, the capacitor and added a treble bleed mod. Now the thing is buzzing when connected. Am I right in thinking that I haven't been too thorough with the grounding?

Sounds like it. Does the buzz stop (or at least reduce) when you touch the strings or bridge?

Yeah it does. The buzz is most significant with distortion added. Not so "present" when playing clean.

Well at least the ground wire from the bridge is properly connected. Don't know what else to suggest, I'm afraid....

Sounds like your house isn't properly grounded. Its pretty common, but still bad.
